This volume contains the findings of an investigation led by Justice René P. Foisy into the causes of a fatal Feb. 1986 collision between a Canadian National Railway freight train and a Via Rail passenger train near Hinton, Alberta. The book first describes the accident scene, both trains, and their crews. The commissioners then examine possible causes for the collision, ranging from technical malfunctions to excessive speed to potential hearts attacks. In particular, the text comments on CN Rail regulations that encouraged a culture of overwork amongst employees, as well as on engineer Jack Hudson’s health and wellness. Throughout, the commission makes recommendations for rectifying sources of mechanical, regulatory, and employer-employee concerns that serve as potential safety hazards.
